Personal Injury Protection and MedPay
Dr. Sundgren provides rehabilitative treatment for injuries sustained in car accidents. All car insurance policies are accepted including insurance purchased outside of Gainesville, FL. Multiple therapies are used to speed and improve recovery. Therapies include manual therapy, therapeutic stretches and exercises, physical therapy modalities, dietary and supplemental nutritional counseling, self care counseling and home therapies, and durable medical equipment. Treatment is continued until preaccident status or maximum medical improvement is reached regardless of PIP insurance limits. Treatment can be provided for both at-fault and not-at-fault accident injured individuals. With a signed Letter of Protection, the patient out-of-pocket expense for treatment including the insurance deductible can be paid upon conclusion of treatment and settlement of the insurance claim. Patients may qualify for discounts described below for their out-of-pocket expenses. Treatment is provided for a range of accident symptoms. This can include neck pain, headaches, sleeplessness, fatigue, upper back and shoulder pain, arm and hand tingling and weakness, lower back pain, leg and foot tingling and weakness, and other symptoms. Manual therapy and rehabilitation can be provided for disc injuries. Treatment appointments typically take between 50 and 70 minutes. The intake appointment typically takes between 2 and 3 hours. BCBS
Dr. Sundgren is an out of network provider for BCBS. He serves both Florida and out of state BCBS enrollees. As an out of network provider Dr. Sundgren is able to provide a variety of health care services to his patients. Services include manual therapy, nutritional counseling, physical therapy modalities, therapeutic procedures, and self care counseling. Patients play an active role in determining what health care treatment approaches are utilized. With a signed Assignment of Benefits, patients are only responsible at the time of service for their coinsurance percentage amount. This coinsurance amount is usually 30-40% of the treatment fees and is the total out-of-pocket expense for patients. The Assignment of Benefits requires that all BCBS statements and payments be sent to Dr. Sundgren. The patient out-of-pocket expense before discounts for the average intake assessment is $185 to $333 and for the average treatment appointment is $91 to $126. Discounts are described below. Treatment appointments typically take between 45 and 60 minutes and the intake appointment between 90 and 120 minutes. Self Pay
Dr. Sundgren provides a self pay plan based upon a flat hourly rate of $160 plus record keeping fees. The self pay out-of-pocket expense before discounts for the average intake assessment is $205 to $365 and for the average treatment appointment is $131 to $171. Discounts are described below. Treatment and intake assessment appointment lengths are the same as for BCBS appointments.
Discount and Bonus Programs
Dr. Sundgren provides several discount programs to reduce the patient out-of-pocket expense for treatment regardless of type of payment. These discounts apply only to the patient out-of-pocket expense and are not applicable to the insurance portion. Dr. Sundgren offers a Health Action Incentive discount up to 25% for healthy behaviors such as documented exercise and good nutrition. Dr. Sundgren offers a student discount of 5-10-15% for part time, full time, and athlete students. A Sliding Scale discount plan is available for those in households falling below 201% of the poverty level. All discounts may be applied to services only. For account bonuses, patients can make prepayments into a Health Investment Account to earn up to 35% additional credit that can be used for both services and products.
